Our Medical Case Managers specialize in the treatment of individuals who have suffered Traumatic brain injury, Spinal Cord injury, Amputation, Burns, and Severe Fractures. We assess, develop, implement, coordinate, monitor and evaluate a comprehensive plan of care through an interdisciplinary team process in conjunction with the client and family. We coordinate and facilitate physician recommendations for each patient. We provide quality customer service and provide verbal and written reports. As a case manager, we assess, monitor, and evaluate the client's personal and medical history, which includes current status, diagnosis, prognosis, current treatment plan, and care provider's level of expertise. We educate the patient and family so that they may make educated decisions related to the medical treatment recommendations. Communication with patient, family, medical staff, caregivers, and the insurance companies is a necessity for appropriate teamwork. We assess and evaluate quality of care and cost effectiveness utilizing cost comparison. The case managers establish realistic goals to achieve optimal outcomes for the patient/client. As we determine specific objectives we determine short and long term rehabilitation goals. We identify actions to meet the goals and target dates for achievement of goals.
Our medical Case Managers assist the patient/client to procure servcies and serve as an advocate on behalf of the client (as indicated) and his/her family. We also act as a liaison to the health care providers and community health resources.
