Coordination of Rehabilitation Services

As our Medical Case Managers assess and develop a treatment plan, we will implement and facilitate the physician recommendations. We then coordinate and monitor the treatment. The case manager also evaluates and identifies potential challenges or complications in physiological, psychological and social function. We also identify potential difficulties in the community reintegration as appropriate. It is our job to know equipment and treatment resources in the patient's community.
